The minimum useful fields

Record the dog’s identity and microchip, vaccine name, date administered, veterinarian or clinic, and any certificate or lot detail the clinic provides. Keep the original record attached. Mark a planned dose as planned, never completed. If you inherited an incomplete history, ask the clinic what can be verified instead of guessing from age.
